GUIDELINES (a) Type clearly; (b) Previous recipients may not be nominated for the same effort for which they have been previously recognized (list available if needed); (c) Anonymous nominations will not be considered; (d) Multiple nominations for 1 person does NOT affect committee’s decision; (e) Nominees are encouraged to attend breakfast, and recipients will be notified ahead; (f) Use concrete examples of activities and services demonstrating how their efforts have helped improve our Community; (g) Be specific, the selection committee can only use written information provided on this form for evaluating who and what initiatives should be recognized. Personal experience or background knowledge by the committee is not permitted.
